REAL AUTO GLASS OF FLORIDA LLC ADAS CALIBRATION WAIVER FORM
After windshield replacement, the forward-facing camera (ADAS – Advanced Driver Assistance Systems) may lose its original factory alignment, affecting the proper operation of safety systems.

These systems include, but are not limited to:

Automatic emergency braking

Lane departure and lane keeping assist

Forward collision warning

Pedestrian detection

Adaptive cruise control

Calibration is the technical procedure recommended by the manufacturer to restore proper operation.

CUSTOMER STATEMENT

I acknowledge that I have been clearly informed that:

Failure to calibrate may cause serious safety system malfunctions.

The vehicle may fail to brake when needed or brake unexpectedly.

Operating without calibration increases the risk of accidents, property damage, serious injury, or death.

The vehicle may operate outside manufacturer specifications.

Insurance companies and manufacturers may deny coverage related to ADAS failures.

Despite understanding these risks, I choose NOT to proceed with calibration at this time and assume full responsibility.

I hereby release Real Auto Glass of Florida LLC from any legal, civil, or financial liability related to the ADAS system.
